2008 Registration Forms
Right click on the icon to download the 2008 QYC YSP Registration Forms. Please remember that Massachusetts Law requires that we have a copy of a camp or school physical and your child's immunization record before they can participate in the program.

2008 YSP Student Handbook
Download the 2007 YSP Student Handbook here! The 2008 Handbook will be available around May 15, 2008. Most policies and procedures are the same as in 2007.

Student Handbook Receipt
Both student and parent/guardian need to sign this Receipt for the Student Handbook. You may bring it to the Orientation on June 21 (Noon to 2 pm) or should you miss the orientation, you may bring it on the first day of your class.

Student Safety & Discipline Policy
This document details safety and behavior expectations and the YSP Discipline Policy, which is approved by the QYC Board of Directors. It must be signed by the student and parent/guardian before the student participates in the Youth Sailing Program.

Commuting / Pick up Permission
If your child will commute to/from the YSP by foot or bike, OR if someone other than the parent/guardian will pick up your child from the program, we must have a signed copy of this form on file.

Alergy Medication / Inhaler Permission
QYC YSP does NOT administer medications. If you deem your child competent to self-administer medication for alergies or asthma, we must have a signed copy of this form on file. PLEASE NOTE: All epi-pens and inhalers must be in waterproof containers and attached to your child's PDF in such a manner that it CANNOT become entangled in Sailing gear.

Childcare / Staff Transportation Permission
Our staff are not allowed to provide childcare or transport your child unless we have a signed copy of this permission form on file.
